Defeating the King in the Mists offers a chance to obtain the belt, but it's not guaranteed. The drop rate is approximately one in five attempts.
Other possible unique drops from the King in the Mists include:
- The Burden of Shadows Chiming Staff
- Beetlebite
- From Nothing
- Pragmatism
The King in the Mists' difficulty scales, increasing the drop chance at higher tiers. His attacks are similar to the Act 1 King in the Mists, offering a degree of familiarity.
